Performer/Presenter Forms

Performer/Presenters must submit the following three forms online.

• Biography Form (All performers must submit (or resubmit) bios for themselves, their accompanying musicians and for living composers whose works they are performing.)
• Program Form

The following form cannot be submitted online. Please print the form and follow the directions.

• Agreement to Perform and Present 
  (must be printed, signed, and mailed to address on form by March 1, 2012)
